Fillable Steps to Health SNAP-Ed Partnership Form FY24  – the purpose of this form is to establish cooperative efforts between NCSU-STH and the partnering agency. These forms must be filled out every year and submitted to sth-administration@ncsu.edu.

PSE Reporting Directions

The reporting directions provide information on: what qualifies as low-income for Steps to Health PSE work, what to report, when to report, and how to find the reach. Reporting should be provided to the RNEA or Tammy Brunelle at tchaseb@ncsu.edu

*Please note, Steps to Health does not have to fund a project or provide services for it to be reported. Individuals should be seen as consultants and technical advisors for creating PSE changes that benefit low-income households and communities, rather than the provider of services. 

PSE Catalog

This catalog is intended for use by agents and educators to understand the specific resources and promotional materials that are available through Steps to Health for use with PSE work with SNAP-Ed eligible sites. Sample items and templates are included in the list below. 

Previous Custom Steps to Health Signs agents have made can be found online via Google Drive.

PSE Menu of Service

The PSE Menu of Services can be used to help guide discussions with partners. There are six versions to choose from, one version is a combination of all the services we can offer and the remaining are broken up into specific settings such as schools or farmers markets. 

Steps to Health Eligibility Confirmation Letter – this letter can be used at sites that qualify for Steps to Health programming are not site eligible.
